Human fibrinogen
Indications/Uses
Therapy & prophylaxis of haemorrhagic diatheses in: Congenital hypo-, dys- or afibrinogenaemia; acquired hypofibrinogenaemia resulting from disorders of synthesis in cases of severe liver parenchyma damage, increased intravascular consumption (eg, as a result of disseminated intravascular coagulation, hyperfibrinolysis), increased blood loss.
Dosage/Direction for Use
IV infusion/inj Max rate: 5 mL/min. Individualized dosage depending on severity of the fibrinogen deficiency, location & extent of bleeding & patient's clinical condition. Normal plasma fibrinogen level: 1.5-4.5 g/L. Prophylaxis in patient w/ congenital hypo-, dys- or afibrinogenaemia & known bleeding tendency Raise fibrinogen levels to 1 g/L; maintain fibrinogen at this level until haemostasis is secure & >0.5 g/L until wound healing is complete. Treatment of bleeding Adult Initially 2 g (30 mg/kg) for perioperative bleeding, w/ subsequent infusions as required. Large amount (4-8 g) may be required in case of severe haemorrhage ie, obstet use/placental abruption. Childn Dose is according to body wt & clinical need, usually 20-30 mg/kg.
Contraindications
Hypersensitivity. Manifest thrombosis or MI, except in cases of life-threatening haemorrhage.
Special Precautions
Immediately stop administration in case of allergic or anaphylactic-type reactions. Risk of thrombosis, particularly w/ high dose or repeated dosing. Caution in patients w/ history of coronary heart disease or MI, patients w/ liver disease, in peri- or post-op patients, in neonates, or in patients at risk of thromboembolic events or disseminated intravascular coagulation. Patients on a controlled Na diet. Possibility of transmitting infective agents. Appropriate vaccination (hepatitis A & B) should be considered for patients in regular/repeated receipt of products from human blood or plasma. Lactation.
